Pick-up Authorization - to authorize a new person to pick up your child on all days or one day only; or to remove someone from your authorization list.
 

Schedule Change Request - to request a change in your child's schedule (more days, less days)
 

Drop-in Request - for playschool only, to request to drop in on a day that your child is not regularly scheduled for
 

Information Update - to update your personal information (address, phone number, email, etc.).  By law, we need to have your current information.
 

Program Withdrawal - to withdraw from our program.
 

Expense Claim - to submit a claim for expenses incurred doing your Family Duty
